If your pet has been seen by the doctor at LCAH within the past six (6) months and you need a refill of your pet's prescription, please submit the form below. Since prescriptions need to be authorized by your pet's doctor, please allow 24 to 48 hours for your refill to be processed. 

A member of our staff will contact you when the refill has been completed.
